Medicare for Therapists: Should You Opt In or Opt Out?

Opting in can expand your client base, but it comes with trade-offs. How to decide what fits your practice.

By Melissa Owen Norris

One of the most common questions we hear is: “Should I opt in to Medicare?” It's not a simple yes or no. The decision affects your client base, finances and administrative workload.

What opting in means

  • Bill Medicare directly for covered services.
  • See clients who rely on Medicare benefits.
  • Be listed as a recognized Medicare provider.

Benefits

  • A large and growing population of eligible clients.
  • Improved accessibility for older adults and people with disabilities.
  • Steady, predictable reimbursement.

Challenges

  • Rates are often lower than commercial payers.
  • Documentation and audit standards are strict.
  • Opting out is a two-year commitment, and switching takes planning.

Opting out

Formally opting out lets you see Medicare beneficiaries privately using a signed private contract. Simply not enrolling does not. You cannot charge Medicare beneficiaries privately for covered services without a formal opt-out.
